The C-Mill is a treadmill that uses visual and acoustic cues for training and evaluation of impaired gait and balance. Why Use the C-Mill?

The ability to adjust your gait to the requirements of the environment is related to fall risk. Someone must be able to avoid a doorstep, puddle of water or other obstructive objects in order to walk safely in daily life. The C-Mill is developed to train these task specific gait adaptations by projecting visual cues on the walking surface. This results in a context specific environment to train and evaluate gait pattern and gait adaptability.

Adjust Gait Pattern

A therapist can decide to adjust a patients gait (e.g. step length or step width) by projecting cues on the belt of the C-Mill. The cues automatically appear based on a patient’s gait. The therapist can influence this pattern by changing the distance between the cues, thus altering and training the gait pattern.

Obstacle Avoidance

A typical obstacle avoidance session on a C-Mill starts with letting the patient walk slowly on the treadmill. Pre-programmed objects of various sizes appear exactly on the position of either left or right foot contact. In addition, the number of steps to respond before an obstacle appears can be set by a therapist, in this manner obstacle avoidance strategies can be trained.

Assessment and Analysis

The C-Mill offers flexible treatment and evaluation protocols without the need for attaching markers or wires to the patient, thereby saving precious treatment time. The CueFors software shows gait parameters like step length, width, frequency and symmetry on a step to- step basis. A report of a training session is automatically generated.

CueFors

CueFors is a software package developed to operate the C-Mill, it facilitates the functionalities for the training of a patient and analyse the corresponding results. While a patient walks on the C-Mill, an operator can control different context specific projection types like stepping stones and obstacles. These projection types can be projected relative to a patients gait in order to train a patients gait adaptability. All results like spatial, temporal and force parameters are automatically stored and can be analysed and exported.

The C-Mill was designed in collaboration with the rehabilitation centers Reade and Heliomare, the Move Research Institute Amsterdam and Forcelink. With complementary scientific, clinical and technical expertise the C-Mill project became a scientific and clinical success.
